What an ingrowing toenail actually is

The edge of the nail presses into, or pierces, the skin at the side of the toe. It is most common on the big toe. The result is a spot that is tender to touch, sore in shoes, sometimes red and swollen, and out of all proportion to its size.

The usual causes are nails cut too short or curved down at the corners, footwear that presses on the toe, a knock or a stubbed toe, or simply the shape of the nail you were born with.

Conservative treatment

AssessmentThe toe is examined and the cause identified — because a nail that keeps coming back needs a different answer from a one-off.
Relieving the pressureThe offending spike of nail is carefully removed and the nail edge eased and smoothed, which is usually where the relief comes from. The area is dressed where needed.
Aftercare and preventionHow to cut it — straight across, not down into the corners — what footwear is doing to it, and a short follow-up review if it needs dressing again.

Ingrowing Toenail — your questions

Will the treatment hurt?
The toe is already sore, so there is some discomfort — but removing the spike of nail is what stops the pain, and most people feel markedly better before they leave.
Will I need surgery?
Usually not. Conservative treatment resolves most ingrowing toenails. If yours keeps coming back, you will be referred rather than treated on repeat.
My toe looks infected — can you still see me?
Book the appointment, and contact your GP as well. Signs of infection include spreading redness, heat, throbbing or discharge. The toe can still be assessed and made comfortable, but antibiotics have to come from your GP.
How do I stop it happening again?
Cut straight across rather than curving down into the corners, do not cut too short, and give the toes room in your shoes. Those three things prevent most recurrences.
